APPLICATION FOR CERTIFICATE COMBINATIONS OR RE-ISSUES
The following documentation must be attached in order for the application to be processed:  A copy of your ID document or Birth Certificate, copies of all relevant Statements of Results or Certificates received and a copy of the receipt received for payment upon application.
